But what is the reality? Is it actually possible to improve on what nature gave you? Can you really extend your "old persuader"? Ways you might try to get bigger! 1 Penis enlargement pillsFirst off, let's deal with the penis enlargement pills. The problem here is that it's always been easy to sell stuff where there's a strong demand for it, whether or not it actually works. From the days of the snake oil salesmen to the days of the Internet websites offering penis enlargement pills, not much has changed: find a willing audience who want to believe that anything is possible, and you'll sell your product. And we live in a world where the quick fix is the attractive option: just imagine - taking a pill to enlarge your penis! Wow! No wonder they sell. Sadly, however, they don't work....but you probably knew that anyway. Dr David Ralph, a consultant urologist at the Lister Hospital, London, said in a review of penis enlargement techniques featured in Men's Health magazine, "If there really was a pill that could enlarge your penis, don't you think one of the major manufactures would be selling it?" And it's a simple one - you basically take your semi-erect penis and force blood into the erectile chambers by running your thumb and fingers down the length of the penis from base to tip. This supposedly forces the blood into the chambers and causes them to expand, rather in the way that skin will stretch when it is put under a constant gentle load. The idea is that you put your old man in a Perspex cylinder and pump the air out. The resulting vacuum causes blood to flow more enthusiastically into the penis, making it swell and enlarge. Over a period of time, the enlargement becomes permanent and the tissues expand. At least, that's the theory. In practice, it does indeed make your penis swell up: the only catch is that when you pull your penis out of the tube, the swelling rapidly goes down. The important question, though, is whether or not it makes your subsequent erections any bigger? Will she (or he) swoon with amazement when she (or he) sees your huge todger? The best answer seems to be - yes, but only during the period while you are regularly pumping. This leaked blood can also clot under the skin, this resulting in a lumpy appearance." Ouch! So, the first thing you need is a pump that regulates the pressure so you don't damage your penis. Surely that offers a means to enlarge the penis? Well, penis enlargement surgery has a somewhat dubious history, having been marked out by some unhappy incidents with incompetent surgeons in its early days. But does it work now? The traditional method of gaining length is to cut the ligament that attaches the penis to the body, an operation that allows the penis to drop forward and down. This increases the flaccid length somewhat, but it does not actually increase the erect length very much. Sexologist Randy Klein conducted a very detailed study of penis enlargement surgery in which he compared the expectations of men who had this operation with the actual results achieved, and he discovered that they were almost all disappointed with what had been achieved - they expected a post-op erect penis length of between seven and nine inches - but the average was in fact under six inches (most of them started off with erect penises of about five inches in length). What's more, to prevent shrinkage as the tissues heal, it's essential to use a stretching device which preserves the increase in length by stretching the ligament and preventing the tissues healing shorter than they were before the operation. To increase girth, the traditional technique was to take fatty tissue from the patient's own body, and insert it into his penis. This could be done with either fat cells or a graft made up of fatty tissue and skin from the buttock crease - in either case, the results tended to reduce over time as the fatty tissue was reabsorbed by the patient's own body. Does this all mean that penis enlargement surgery is useless? Well.....that is a hard question to answer. There are some specialists on the internet who claim high success rates. Dr Douglas Whitehead, for example, does not use either of the two methods mentioned above to increase girth. Instead, he employs a newer technique using grafts of specially treated tissue extracted from tissue donors, and surgically placed around the patient's penis to increase length and girth. This is called the allograft dermal matrix technique. He also cuts the penile ligaments in a way that leaves much less scarring, though he still advises the use of a stretching device to maintain the gains in penis length after the operation. But the $64,000 question is: What is the average increase in length and girth? To quote from his website: "Average expected length gain in the flaccid state is 1 inch. Well motivated patients may gain up to 2 inches. After allograft dermal matrix graft widening, the penis does not shrink to its original small flaccid length, but stays slightly longer. Usually there is a slightly smaller penile length gain in the erect state than in the flaccid state." He goes on to say: "We have noticed a permanent increase in penile girth (circumference) in the flaccid state with our...allograft dermal matrix graft technique. While a 40 to 50% gain in girth (flaccid state) may be achieved, most men get an increase of about 30%. An approximate 10 to 15% gain in girth is obtained in the erect state." Now, the only way a man can assess whether the risks are right for him is to weigh up the evidence and make a decision. But with some of the newer surgical penis enlargement techniques these risks have not yet been defined, as Dr Whitehead admits on his website. Another problem is that if you look at websites by penis enlargement surgeons, you may find contradictory or conflicting information - and it's all presented with great authority, which makes it well-night impossible for the layman to find out what the facts really are. Dr Stephen Giunta, another American surgeon, uses one of the two traditional methods of increasing girth mentioned above, which were, as you might recall, free fat injection or dermal grafts of fat and skin, in both cases taken from the patient's own body. Dr Giunta's method of choice is the free fat injection technique, of which he says: "Despite what you may have heard or read about Free Fat Transfer (FFT) - it doesn't work, it disappears, it leaves cysts or nodules, or 'lumpy-looking,' distorted penises - this simply isn't true anymore if FFT is done correctly." Dr Giunta uses free fat transfer rather than dermal fat grafts for reasons he sets out in some detail as follows: "Let me share a few words about Dermal Fat Grafts (DFG) and AlloDerm. I have seen and repaired many cases where the DFG have twisted, shortened, become infected and have ultimately caused permanent loss in penile length. Unsightly large painful donor site scars have also occurred. Many times it is very difficult to control the size of the thickening with these DFG and, as a rule, you only get one chance at a good result. AlloDerm, recently used by a few surgeons, is a thin sheet of cadaver (a dead person's) preserved and treated skin. The skin is rolled in several layers around the penile shaft for thickening. This procedure has only been used for a couple of years and, to me, seems very cumbersome and expensive. While I have seen a few complications thus far, the final word in AlloDerm is still quite a distance away." By contrast, Dr Gary Rheinschild uses the dermal fat graft technique. He claims that the special skills he uses in shaping and placing these strips, as well as unique post-op care, result in much greater patient satisfaction: "The patient can expect an increase in girth of up to 2 inches. By performing a dermal graft as a second procedure, Dr. Rheinschild has found that there is only a minimal loss in girth [post operatively]. Patients who have their enhancement surgery performed in this manner sustain an aesthetic appearing penis without any significant scarring. This procedure has proven to be far superior to using fat injections, in which the majority of the fat is often reabsorbed and results in distorted and uneven areas with nodules scattered throughout the penis." And the last word must go to Dr Gary Alter: "Dermal-fat graft augmentation has complications of penis shortening, penile curvature, and asymmetry if segments of the graft do not completely survive. Other complications common to all operations can occur such as infection, bleeding, and wound problems. However, deforming complications are uncommon if surgery is performed by a skilled, experienced surgeon. However, at the present time, I am not performing any girth enhancement procedures. The dermal fat graft procedure works very well most of the time and tends to stay permanently; however long term follow-up is unavailable and some problems can occur. Some physicians are using Alloderm (cadaver skin) instead of dermal fat grafts to augment the penis. The Alloderm has the advantage of avoiding the donor scar from the dermal fat graft. Alloderm is promising, but it is too soon to know if it will permanently last. I am waiting for longer follow-up on Alloderm prior to performing the operation." So - who do you believe? Well, that, unfortunately, is a decision that only the prospective patient can make for himself. All of these websites are listed below in the references.
